Complete the following Transportation form that applies to your Field Trip:
- Requires District transport: Bus Transportation Request Form
- Will Be Transported by Volunteers: Volunteer Personal Automobile Use Form English / Spanish
- Trip Does Not Require Transportation: Non-Bus Field Trip

Parent/Guardian Transportation for a Field Trip
If the district is providing transportation to and from the field trip a parent/guardian may choose to transport their own child in their personal automobile. The parent would need to sign their child out in their school site office. Teachers should take a Field Trip Sign Out Form with them on a field trip in the event a parent/guardian decides to transport their child home in their personal automobile. Field Trip Sign Out forms should be given to the attendance secretary upon return from the field trip. If the district offers transportation, parents/guardians do not need to complete the Volunteer Personal Automobile Use Form to transport their own child.
In the case of McCaffrey sporting events, the district does not provide transportation. Parents/Guardians/Volunteers are required to complete the Volunteer Personal Automobile Use Form before they can transport any student(s) that is not their own child.

- When Planning a trip, please consider the grade level of the students who will be transported. School Buses can accommodate the following number of passengers by grade level.
a. K-2nd Grade: 65 passengers
b. 3rd-5th Grade: 60 passengersc. 6th-8th Grade: 54 passengers
